Education Health Care Plan Coordinator | 9581 | Permanent Contract | 37 hours per week | £39,862 to £41,771 per annum | Grade J | Norfolk

Are you an enthusiastic professional, highly skilled in relationship-based practice? If you have experience in the special education sector and a commitment to facilitating positive outcomes for children and young people, we want to hear from you. We have an opportunity to join our highly skilled team within the Family Help & High Needs SEND Service, with a focus on driving forward performance improvement and quality of Education Health Care Plans (EHCPs). As an EHCP Coordinator, you will play a crucial role in navigating the EHCP process, collaborating with stakeholders, and ensuring comprehensive support for individuals.

Responsibilities

  • Collaborating with schools, parents, healthcare professionals, and local authority colleagues to gather relevant information
  • Coordinating and managing the EHCP assessment, review and placement planning process
  • Ensuring that EHCPs are accurate, comprehensive, and reflective of the individual\’s needs
  • Liaising with external stakeholders to ensure the seamless delivery of support services
  • Keeping up to date with legislative changes and best practices in the field

Your role will be challenging but immensely rewarding, as our team works on complex cases covering children and young people aged 0-25 years. To succeed, you will be highly motivated, person centred, well organised and have experience of the SEND field and legal framework.
