We are seeking an experienced and motivated Lead Practice Nurse to join a forward-thinking and supportive primary care team on a 12-month fixed term contract.
This is an exciting opportunity for a clinically confident nurse who is passionate about delivering high-quality patient care while leading and developing a dedicated nursing team.
The Role
As Lead Practice Nurse, you will play a key role in both clinical delivery and leadership within the practice.
Clinical Responsibilities:
- Deliver high-quality nursing care, including chronic disease management (e.g. diabetes, asthma, COPD)
- Conduct cervical screening, immunisations, travel health consultations, and wound care
- Perform health promotion and screening activities
- Manage long-term condition reviews and develop patient-centred care plans
- Prescribe where qualified (Independent Prescriber qualification desirable)
- Maintain accurate, up-to-date clinical records in line with professional standards
Leadership & Management Responsibilities:
- Provide leadership, supervision, and mentorship to the nursing team and HCAs
- Support recruitment, induction, and ongoing development of nursing staff
- Lead clinical audits, quality improvement initiatives, and infection prevention control
- Ensure compliance with CQC and relevant regulatory standards
- Contribute to policy development and continuous service improvement
About You
- Registered Nurse (NMC) with experience in primary care
- Strong background in chronic disease management
- Confident leader with excellent communication and organisational skills
- Passionate about patient care and team development
