We are currently recruiting on behalf of a valued Primary Care client in Reading for an experienced Clinical Pharmacist to join their established multidisciplinary team on a full-time basis. This is an excellent opportunity for a motivated pharmacist with Primary Care experience to play a key role in delivering high-quality patient-centred care while supporting medicines optimisation and prescribing safety within a collaborative clinical environment.
Key Responsibilities
- Provide clinical pharmacy support within a Primary Care setting
- Conduct structured medication reviews and medicines reconciliation
- Support safe, cost-effective prescribing across the practice population
- Work closely with GPs, nurses, and wider MDT colleagues
- Manage long-term condition medication reviews
- Support patients with complex medication needs
- Contribute to prescribing audits and quality improvement initiatives
Essential Requirements
- GPhC registered pharmacist
- Proven Primary Care experience (essential)
- Strong clinical knowledge and patient-centred approach
- Excellent communication and teamwork skills
- Ability to work independently within a multidisciplinary environment
