We are currently recruiting on behalf of a valued Primary Care client in Reading for an experienced Clinical Pharmacist to join their established multidisciplinary team on a full-time basis. This is an excellent opportunity for a motivated pharmacist with Primary Care experience to play a key role in delivering high-quality patient-centred care while supporting medicines optimisation and prescribing safety within a collaborative clinical environment.

Key Responsibilities

  • Provide clinical pharmacy support within a Primary Care setting
  • Conduct structured medication reviews and medicines reconciliation
  • Support safe, cost-effective prescribing across the practice population
  • Work closely with GPs, nurses, and wider MDT colleagues
  • Manage long-term condition medication reviews
  • Support patients with complex medication needs
  • Contribute to prescribing audits and quality improvement initiatives

Essential Requirements

  • GPhC registered pharmacist
  • Proven Primary Care experience (essential)
  • Strong clinical knowledge and patient-centred approach
  • Excellent communication and teamwork skills
  • Ability to work independently within a multidisciplinary environment
