Weare looking for Health care support workers to join our community nursing teamwith a number of vacancies countywide.
Main duties of the job
Successful applicants will be required to provide nursing care to a variety of patients within their own home or livingenvironment which will include insulin administration, wound care, samplecollection ie venepuncture , recordingobservations plus many other aspects of patient care. This job entails loneworking.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Person Specification
Experience
- Experience of working within multidisciplinary teams Experience of working with adults. Relevant assistant experience .
Qualifications
- Relevant NVQ Level 3/Level 3 Diploma in Health & Social Care or equivalent relevant experience.
- GCSE equivalent in English and Mathematics.
- Knowledge of Microsoft Packages
- An understanding of County Durham and Darlington NHS Foundation Trust, its culture, strategic direction, achievements and NHS standing
Special Skills & Knowledge
- Ability to communicate clearly and effectively Able to work as part of team
- Able to observe and record physical and social changes in patients Able to identify when to report to a registered nurse / therapist Able to work alone with patients within an agreed care plan Able to work with minimal supervision An appreciation of dealing with people in their own homes
- Demonstrate the ability (with training if necessary) to undertake basic clinical activities such as venepuncture, simple wound dressings, blood glucose monitoring (this list is not exhaustive) and other clinical duties identified by registered nurse Able to maintain Confidentiality
- Understand of working with other agencies/multi-disciplinary working.

Special Requirements
- Able to travel across Trust sites to meet the requirements of the post
