We have an exciting opportunity for band 5 staff nurses to join our brand new Hospital at Home service. This is a thrilling chance for a highly motivated staff nurse to join an expanding team and develop autonomous practitioner skills while caring for our frail patients within the community. The service will be based out of the acute hospital site and will work closely with our Acute Frailty Teams to identify patients suitable to be cared for in their own homes. The service will provide 7 days cover and a full UK driving licence is required with access to a car; business mileage is paid.

Main duties of the job

  • To assess patients’ needs and develop, implement and evaluate programmes of care.
  • To organise the delivery of nursing care, deploying staff effectively.
  • To be aware of, and communicate to team members, changes to patient information and treatment plans, both verbally and in writing.
  • To adhere to NMC standards of record keeping.
  • To assist and support in home setting, monitoring and maintaining standards of care.
  • To participate in nursing audit and service improvement activities e.g. Essence of Care.
  • To be aware of resource constraints and the importance of working within budget.
  • To provide advice and act as role model to other members of the team.
  • To be proactive in managing risk and maintaining patient safety.
  • To supervise the work of nursing support staff, ensuring tasks undertaken have been delegated appropriately.
  • To contribute to the day-to-day management of the Directorate, co-operating with colleagues to provide an appropriate level of nursing service and to address bed management issues.
  • To participate in education and training of junior staff.
  • To involve patients/carers in the planning and delivery of care.
  • To assist in resolving conflict and defusing situations where appropriate.
  • To promote a positive and professional image of nursing at all times.

Person Specification

  • Statutory Registration: Registered Nurse (adult or Child) on part 1 of the NMC register, Valid UK NMC PIN.
  • Qualifications: RGN Educated to degree level or equivalent experience, Teaching qualification, Care of the Elderly modules/training.
  • Experience: Clinical knowledge of acute nursing, Literate and articulate, Good communication skills, Evidence of undertaking and developing audits to inform practice, Knowledge of relevant nursing and health care policies.
  • Special Skills and Knowledge: Current knowledge of professional developments, H@H experience, Community care experience.
  • Special Requirements: Patient Focused: Understands how their role impacts on the patient journey, and a willingness to place the patient at the centre of what they do. Achieving Results: Experience of successfully working to deadlines and completing tasks. Inspirational Leadership: Keen to develop themselves and others, open to new ideas and willing to share their experience with others. Maximising Value: Takes ownership of problems and does not overly complicate matters or involve colleagues unnecessarily. Working Together: Approachable and able to build relationships with a wide variety of colleagues. Full UK Driving licence, Willingness to use own car.

Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: £31,049 to £37,796 a year per annum, pro rata.
