We have an exciting opportunity for a qualified Dietitian to join our Community team. County Durham & Darlington NHS Foundation Trust is an excellent place to live with beautiful countryside, coastlines and nearby cities. It offers excellent transport links to Newcastle, Sunderland and other UK cities. The trust provides care from two acute sites, a centre for planned care, and many community hospitals and home settings.

Main duties of the job
The Nutrition and Dietetic service is split into speciality teams: secondary care, bariatrics, primary care, adult and paediatric diabetes, home enteral feeding, and paediatrics. We are supported by dietetic assistants and central administration. The service is based on four sites at Darlington Memorial Hospital, University Hospital North Durham, Bishop Auckland Hospital and Shotley Bridge Hospital. The community dietetic team provides cover to the general adult population within County Durham and Darlington and covers nursing home visits, domiciliary home visits and community training sessions.
Job responsibilities
The post holder will be responsible for their own caseload referred to the service by community or secondary care professionals. Patients will be seen individually, in groups or with carers in a variety of settings including hospital wards, outpatient clinics, nursing homes, own homes and telephone clinics. The post holder will work closely with other Community Dietitians and members of the wider multidisciplinary team.
The department trains dietetic students (A, B, and C placement) and the post holder will have an active role in student training planning, coordination and assessment. New graduates are very welcome to apply for this training and development role, with responsibilities and role adjusted accordingly. The appointment will be made at Band 6 Annex 21 (75% of top of band 6 as per NHS Agenda for Change) or Band 6 dependent on qualifications and experience. If appointed under Annex 21, completion of the 12‑month competency based training programme allows progression to a permanent Band 6 grade.
Person Specification
- Previous experience of working in a variety of clinical settings
- Experience of working within MDT environments
- Previous experience of audit and research including submission / presentation to journals / conferences
- Previous experience of working within primary care

Special Skills & Knowledge
- Completion of postgraduate specialist education e.g. PENG course or other
- Ability to plan, initiate and evaluate new services and manage change
- Highly developed understanding of the changing NHS environment and its impact on service provision and staff
- Understanding of local and national strategies and policies relating to nutrition, health and social care as relevant to community dietetics
- Member of BDA
- Experience of using SystmOne
Qualifications
