We are looking for applicants with experience in acuteand/or general internal medicine who also have excellent team-working skillswith the abilities and professional skills to work well in a multi-professionalteam. The posts would ideally suit a post CMT level doctor who wishes to gainfurther experience and skills before considering their next career move, or aregistrar grade clinician who is keen to enhance their skills within themanagement of the acutely unwell adult patient. Each post holder will have asupervisor and encouraged to enrol onto a generic E-Portfolio.

Based at University Hospital North Durham, thepost-holder will be a key senior member of the medicine team with a clear rolein ensuring comprehensive ongoing care for their inpatients. They will beworking primarily with in the Integrated Medical Speciality under thesupervisions of Medical Consultants based in a speciality but with flexibilityto move within the remit of general medicine. As part of a multidisciplinaryteam, you will be committed to delivering clinical and quality targets agreedboth nationally and locally with the clinical teams and commissioners.

Main duties of the job

1. Liaise between nurses, junior doctors, patients,relatives and senior medical staff.

2. Attend and participate in ward rounds as timetabled

3. Attend outpatient clinics.

4. Study for higher examination and maintain continuedprofessional development.

5. Attend weekly educational and multidisciplinary sessions.

6. Undertake audit at various times throughout therotations.

7. To supervise and manage junior medical staff

8. To assist in the teaching and training programmes forJunior Staff and Postgraduate Medical Education.

9. To support the development of clinical governance, usingaudit findings to improve the quality of treatment and to minimise clinicalrisk.

10. To develop innovative working practices, andto support the growing clinical contribution of non-medical staff.

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients\’ homes.

THE TRUST

On the 1 April 2011, County Durham and Darlington NHSFoundation Trust came together with Community Services to form a new integratedhealth care organisation delivering integrated acute and community healthservices to patients committed to providing quality care along the wholepathway; at home, in the community and in hospital.

We provide services from three main hospital sites, sixcommunity hospitals, and around 80 locations across the area plus the caredelivered at home.

Our range of services includes hospital based emergency andplanned care, including surgery, medical treatment for serious andlife-threatening conditions, paediatrics and women services, community basedservices for long term conditions like diabetes or respiratory illness; and healthand well-being services to improve health and prevent illness, such as smokingcessation and alcohol reduction. So now, whatever the health care needs of ourcommunity, and wherever they need them in County Durham, our commitment is toprovide these to the best standard and quality.

We provide acute services, for the most seriously ill orinjured patients through our Accident & Emergency departments at DarlingtonMemorial and University Hospital of North Durham. Planned patient care,rehabilitation and recovery, and an urgent care centre are provided at BishopAuckland Hospital. Outpatient and diagnostic facilities are provided at allthese sites.

We aim to treat patients as close to home as possible soprovideoutpatient and community facilities, including inpatients and day careat six community hospitals.

The Trust in its aim to continue to drive the quality ofservice provided to the community is to develop greater interdependence betweenour hospitals by creating centres of excellence in a range of specialties oneach site, and greater integration between hospital and community services.

Recent developments in this area includes the approval tocentralise hyper-acute stroke services on a single site, at University Hospitalof North Durham allowing us to improve emergency stroke care during the first72 hours and the development of a specialist bariatric service at Darlington.

University Hospital of North Durham has 366 beds andprovides a wide range of acute specialties including Accident & Emergency,Anaesthetics and ITU, General Medicine, General and Vascular Surgery, Urology(Day case only), Orthopaedics, Obstetrics & Gynaecology, Paediatrics,Plastic Surgery, Rheumatology, Dermatology and Genito-Urinary Medicine.Additional out-patient facilities exist for ENT, Nephrology, Neurology,Radiotherapy/Oncology, Ophthalmology, Oral Surgery and Plastic Surgery.Clinical support departments of Imaging (including PACS digital radiology, CT,MRI and ultrasound), Coronary Angiography, Medical Physics, Pathology, Pharmacyand Allied Health Professions are all on site.

Recent approval will see Durham provide a hyper-acute strokeunit, supporting 24/7 emergency stroke care, including administration ofthrombolysis where appropriate, on a sustainable basis

The Hospital was built under the Private Finance Initiativeand opened in 2001, replacing the former Dryburn Hospital that has beendemolished. It serves a population of c250, 000. For Plastic Surgery andDermatology the population served is c750,000, covering Sunderland and SouthTyneside as well.

The Department of Medicine is part of the acute andlong-term conditions care group and provides all major medical specialties.There are 220 beds in the University Hospital for medical specialities, with afurther 68 rehabilitation beds in satellite hospitals. There is a full range ofdiagnostic support with access to CT, MRI and Ultrasound including carotidimaging.

There is a 40 bed acute medical unit receivingover 40 admissions/24 hours plus a co-located ambulatory care service. The AMUis supported by acute care physicians. There is a dedicated 24 bedded hyperacute stroke unit with support from Stroke consultants with direct admissions24/7 for all acute stroke admissions.
