CDDFT community services areoffering an exciting opportunity for a registered nurse to join the team as a9-month secondment opportunity covering maternity leave based in Bishop Auckland supporting patients living across Durham Dales, Sedgefield and Darlington suffering a variety of respiratory conditions including COPD, Emphysema, Asthma, Bronchiectasis and Pulmonary Fibrosis.

The post is part time,24.5 hours a week. The service is open Monday-Friday 9am 5pm, hours can benegotiated and flexible over 3 or 4 days.

Main duties of the job

  • Manage a caseload of patients suffering arespiratory condition as part of the wider team supported by Respiratory Specialist Nurses.
  • Patient education supporting self-care andmanagement including proactive exacerbation support, pulmonary rehabilitationand smoking cessation.
  • Medication optimisation and inhaler technique.
  • Support patient support groups, lower band staffmembers plus students.
  • Contribute to creating improvement, transformationand innovation within the service to promote best practice.
  • Commit to continuous personal development/lifelong learning byattending mandatory training, use of e Learning and adhere to all trust policiesand procedures.
  • To understand owns role within the wider multi-disciplinary team andarticulate own developmental needs through attending clinical supervision, 1:1sessions, undertaking appraisal preparation and contributing to objectivesetting and personal development planning.
  • Promote service development, improvements and quality expansion andefficiencies.
  • Support provision for a high quality, value for money and effectiveservice.

  • Toco-manage a case load of complex respiratory patients, under direction of thecommunity respiratory nursing team, across the community respiratory specialistservice, within clinic and patients home environments, Monday-Friday 9am-5pm.
  • To undertakeholistic patient respiratory assessment and review including social andpsychological well-being and progress treatment interventions and adaptinterventions, within set protocols, to progress treatment interventions and tomeet the changing needs of the patient.
  • Develop, implement and evaluate patient treatment plans.
  • Communicatepatient treatment plan changes to respiratory specialist nursing team.
  • Understandbasic respiratory anatomy and pathophysiology and the function and managementof all respiratory conditions including mental well-being.
  • Diagnosticspirometry performance qualification and registration either through The Association for Respiratory Technologyand Physiology (ARTP) or at certified ARTP standard, is desirable but not essential as training will beprovided.
  • Performcapillary blood gas testing and oxygen reviews.
  • Provide longterm condition care following the COPD pathway.
  • Accuratelyrecord clinical data and understand its significance including heart rate andrhythm, respiratory rate, pulse oximetry, peak flow rate, and blood pressure.
  • Complete effective record keeping which demonstrates the decision-makingprocess, care planned and given, as well as patient outcomes in accordance withtrust policy and legal requirements.
  • Understandthe benefits of non-pharmacological interventions for respiratoryillness/conditions including smoking cessation, weight management andnutrition, physical activity and brief cognitive behavioral therapy (CBT).
