Community Sister

This is a dynamic clinical and leadership role within a well‑established district nursing team. The position requires flexibility to work across the Easington North locality, delivering community‑based care to patients with a range of needs, including those who are particularly vulnerable and receiving end‑of‑life care.
The role combines direct clinical responsibilities with management and leadership duties. There is an expectation to work core hours to attend meetings, support communication, and ensure the team remains informed of organisational developments and evolving best practice.
Main duties of the job
- Lead and manage a defined caseload and the staff within the Easington North District Nursing Team.
- Work proactively, demonstrating sound clinical judgment and the ability to make timely, risk‑assessed decisions to support patients across the region.
- Conduct one‑to‑one supervision, complete appraisals, ensure staff compliance with mandatory training, and identify opportunities to develop and enhance the service in response to changing patient needs.
- Manage urgent community response (UCR) duties in a busy and dynamic healthcare setting.

Job responsibilities
- Provide community nursing care to patients throughout the Trust’s footprint, managing complex needs and conducting clinical assessments.
- Deliver patient care and coordinate with the multidisciplinary team and out‑of‑hours services to ensure appropriate and timely interventions.
- Lead, support, and develop team members while assisting the Team Lead, model professional standards, and promote high‑quality, responsive patient care.
- Work predominately between 08:00 and 21:00 across a 7‑day service, including attendance at meetings to remain informed of organisational developments and represent the district nursing team.
Person Specification - Qualifications
- Experience in community nursing.
