We currently have an exciting opportunity for dynamic, highly motivated, innovative and forward-thinking Advanced Nurse Practitioner (ANP) to provide clinical expertise in the assessment, development, planning, implementation and monitoring of patient care with the community hospitals across County Durham. These hospitals provide rehabilitation, step down/up care and palliative and end-of-life services. Priding ourselves with the multi-disciplinary approach for best patient outcomes.
The post holder will require specialist knowledge to demonstrate safe, competent clinical decision-making and expert patient care including diagnostic skills. Be a lead specialist in a defined area of nursing care. Provides education and training to other staff, students and/or patients/clients. Undertakes research and leads clinical audits in own specialist area. This post will be based within one of the community hospitals; however, there is an expectation that the successful candidate will need to travel between community hospitals within County Durham.
Main duties of the job
Working alongside the Medical Practitioners and the Advanced Clinical Practitioner. The post holder would be required to use clinical knowledge and experience combined with sound clinical reasoning to interpret information obtained from either patient, relative/carers or referring source in order to make clinical decisions, provide differential diagnosis and create records to support during consultation period. Then in collaboration with senior decision maker agree most appropriate outcome for the patient and expedite. As the ward ANP you will review and be the prescriber of medications.

You will be strong, motivated and enthusiastic, with excellent leadership skills and the ability to problem-solve effectively. The successful post holder should be an excellent communicator with all stakeholders and positivity and the ability to be proactive is a must for this role. The successful candidate should be proficient in completing EHCPs and treatment escalation plans. The ANP will be an active part of the ward Huddle ensuring plans are in place for each patient. You will also be an integral part of the Medical ward rounds.
Person Specification
- Clinical skills and experience in older persons management and long-term conditions Frailty/older persons care CGA and EHCP
- Ability to work hours flexibly when service needs require
- Ability to travel independently across sites to fulfil the requirements of the post
- Experience working at Nurse Advanced Level or demonstrates ability to progress to this level.
- Excellent Communication skills
Qualifications

- Educated to Degree level or working towards.
- Teaching and mentoring qualification.
- Clinical skills course.
- Non-medical prescribing or a willingness to undertake Long term Conditions Fellowship of older persons care
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
County Durham & Darlington NHS Foundation Trust £47,810 to £54,710 a year per annum, pro rata.
