Join CatholicCare Sydney (CCS) and make a real difference for older people living at home. As a dual-role Care Partner (EEN), you’ll provide evidence-based nursing care and coordinate home care services under the Support at Home Program, partnering with clients and families to deliver person-centred, wellness and reablement-focused care aligned to the Strengthened Aged Care Quality Standards (2025).
About the role
- Manage a caseload and coordinate services, supports and budgets to meet client goals and needs
- Deliver evidence-based nursing care within EEN scope
- Liaise and build strong relationships with clients, families, GPs, allied health and support services
- Maintain accurate documentation and meet performance expectations (KPIs)
- Participate in supervision, training and quality/safety practices
- Maintain accurate documentation, compliance and quality outcomes
- Participate in supervision, education and training
About you
- Diploma of Nursing + AHPRA registration as an Endorsed EN (medication endorsement)
- 2+ years EN experience, ideally in aged care/community care coordination
- Strong assessment, care planning, documentation and communication skills
- Confident with databases/IT, organised and able to manage competing priorities
- Full, unrestricted Australian Driver Licence
- Values inclusion and can work respectfully with diverse communities
- Experience with HCP/Support at Home and/or CHSP
- Dementia, palliative care, allied health coordination or specialised supports exposure
