St Vincent Home is a residential shelter for seniors aged 60 and above who lack family support, have limited financial means, and require stable housing. We offer a socially inclusive environment where seniors can live communally as an extended family, build meaningful connections, and age actively within a supportive community.
Programme Planning
- Conceptualise, organize and run activities and events for residents
- Build rapport with residents and understand their interests, preferences, and limitations
- Collect feedback and prepare reports, evaluations, and summaries of programme outcomes
Medical Support
- Manage medical records, appointment scheduling and transport, and prescriptions for residents
- Monitor residents’ general well-being, including observing changes in mood, behaviour, cognition or physical condition and promptly communicating any concerns to the team or reporting officer
Case Management
- Assist in needs assessment, care plans formulation and follow-ups
- Provide emotional support, financial planning and basic counselling to residents
- Assist in developing and documenting ACP and Afterlife Planning
Administration
- Manage residents’ records and support daily operations
- Liaise with partners, vendors, and external organisations for programme and assistance delivery
- Any other duties, including hospital visits, as assigned by the reporting officer
Requirements
- Related qualification in working with the elderly or in the social service sector
- Experience working with seniors, eldercare, community services, or programmes planning will be an advantage
- Ability to work independently and as part of a team
- People-oriented, good interpersonal and relation building skills, especially with elderly residents
- Problem-solving skills and can-do attitude
- Able to work after office hours when required
Work Arrangements
- 5-days work week, may include weekends and on-call responsibilities
