Catholic Charities of the Archdiocese of Washington seeks a Clinical Care Coordinator to assess functioning and develop individualized rehabilitation plans for adults with mental illness, using the Recovery model. You will lead IRP development with the ACT team, coordinate hospital treatment planning and transitions, and educate clients on ADL and health benefits, while adhering to DMH guidelines.
This role requires:
- A Bachelor's in Social Work
- A DC license
- At least three years of mental health experience
