Catalyst Support is seeking a compassionate Community Connector to join our GP Integrated Mental Health Service and Mental Health Integrated Community Service in Surrey Heath. You will assess needs, provide brief psychosocial interventions, and connect people to services and networks that promote independence and recovery.
In this role you’ll work with GPs, practitioners and partner agencies to deliver joined-up support, develop personal plans, and support community wellbeing.
