Job Role
Catalyst Choices are seeking dedicated and compassionate Support Care Worker/Care Giver to join our team. You’ll play a vital role in supporting individuals with their day-to-day needs, promoting independence, dignity, and wellbeing.
Responsibilities
- Deliver high-quality, person-centred care and support.
- Assist with daily living activities including personal care, meals, and mobility.
- Build trusting relationships with individuals, families, and colleagues.
- Support individuals to engage in meaningful activities and community life.
- Monitor wellbeing and report any concerns promptly.
- Maintain accurate records and contribute to care planning.
- Work collaboratively with the wider care team and health professionals.
