Responsibilities:

  • Provide tailored compassionate care primarily in the homes of individuals and support them to live independently and with dignity within their communities.
  • Deliver high-quality, person-centered care to individuals’ homes and within their communities.
  • Support individuals in maintaining or developing new skills and personal interests.
  • Adapt care and support plans to meet the diverse needs of each individual, ensuring their comfort and safety.
  • Engage in proactive communication with families and care teams to ensure effective care delivery.
  • Support individuals in temporary residential placements or their own homes.
  • Perform household chores such as laundry, cleaning, and food shopping.
  • Manage and support individuals with budgeting and financial management.
  • Assist with all aspects of personal care, including hygiene, dressing, and feeding.
  • Plan, participate in, and budget for community activities.
  • Maintain accurate records and clear communication with the brand team and other healthcare professionals.

Qualifications and Experience:

  • Preferred NVQ Level 3 in Health and Social Care (Level 2 acceptable).
  • Minimum of 6 months of experience in similar healthcare settings and experience in support with complex needs, behaviours that challenge, and autism.
  • Business Car Insurance would be advantageous.
  • Full UK driving license would be preferred.
