Responsibilities:
- Provide tailored compassionate care primarily in the homes of individuals and support them to live independently and with dignity within their communities.
- Deliver high-quality, person-centered care to individuals’ homes and within their communities.
- Support individuals in maintaining or developing new skills and personal interests.
- Adapt care and support plans to meet the diverse needs of each individual, ensuring their comfort and safety.
- Engage in proactive communication with families and care teams to ensure effective care delivery.
- Support individuals in temporary residential placements or their own homes.
- Perform household chores such as laundry, cleaning, and food shopping.
- Manage and support individuals with budgeting and financial management.
- Assist with all aspects of personal care, including hygiene, dressing, and feeding.
- Plan, participate in, and budget for community activities.
- Maintain accurate records and clear communication with the brand team and other healthcare professionals.
Qualifications and Experience:
- Preferred NVQ Level 3 in Health and Social Care (Level 2 acceptable).
- Minimum of 6 months of experience in similar healthcare settings and experience in support with complex needs, behaviours that challenge, and autism.
- Business Car Insurance would be advantageous.
- Full UK driving license would be preferred.
