The predicted salary is between 60000 - 80000 £ per year.

An opportunity has arisen for a GP to work 3.25 or 4 sessions per week on a permanent basis. You will benefit from 6 weeks annual leave and 1 week study leave pro-rata. We also offer in-house educational sessions as well as monthly PLTs (Protected Learning Time) sessions.

Main duties of the job:

  • Provide high‐quality, patient‐centred care within the practice.
  • Work collaboratively with a multidisciplinary team, including nurses, pharmacists, HCA's, mental health practitioners, and social prescribers, to ensure integrated patient care.
  • Conduct consultations, assessments, and follow‐ups with patients.
  • Contribute to the management of chronic disease, health promotion, and disease prevention in line with best practices.
  • Participate in clinical audits, training, and quality improvement initiatives to enhance patient care within the Practice.
  • Triage patients via a total triage model.

Castlefields Health Centre's highly motivated clinical teams consist of: 8 GP Partners, 5 Salaried GPs, 3 ANPs (one specialising in the care of over 75s, another in mental health), 4 Practice Nurses, HCAs/Assistant Practitioner, 2 Clinical Pharmacists, SCIP Worker, Community Matron & we embrace Social prescribing with the wonderful Wellbeing Enterprises. We are proud to consistently excel with QOF & participate in many enhanced services. We are a training Practice with three of our GP Partners being GP trainers. Job responsibilities:

  • The post-holder will make themselves available to undertake a variety of duties including face-to-face surgery consultations, telephone consultations and queries, visiting patients at home, possible weekly ward round to our aligned care home, checking, authorising and signing repeat prescriptions (EPS) and dealing with queries, paperwork and correspondence in a timely fashion along with triaging patients via our total triage model.
  • Making professional, autonomous decisions in relation to presenting problems, whether self‐referred or referred from other health care workers within the organisation.
  • Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
  • Screening patients for disease risk factors and early signs of illness.
  • In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
  • Providing advice and health education.
  • Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Compiling and issuing computer‐generated acute and repeat prescriptions (EPS), avoiding hand‐written prescriptions whenever possible.
  • Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.

Other Responsibilities:

  • Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health, and safety.
  • A commitment to life‐long learning and audit to ensure evidence‐based best practice.
  • Contributing to evaluation/audit and clinical standard setting within the organisation.
  • Contributing to the development of computer‐based patient records.
  • Contributing to the summarising of patient records and coding patient data.
  • Attending training and events organised by the practice or other agencies, where appropriate.
  • Contributing to teaching where appropriate.

Person Specification:

  • Qualifications - A medical practitioner whose name is included in the General Practitioner Register under article 10 of the General and Specialist Medical Practice Order 2003 - Not subject to suspension under section 41A of the Medical Act 1983.
  • Qualified General Practitioner (completed certificate of Completion of Training CCT).
  • Currently on the national performers list and not suspended from that list or from the medical register.
  • DBS Enhanced Disclosure.
  • Have had an annual NHS appraisal.
  • Evidence of further postgraduate educational activities in relevant fields.
  • MRCGP DCH DRCOG RCGP Substance Misuse Certificate Level 1 or 2.
  • F2 supervisor training/teaching diploma (this is essential for HDS).
  • DFSRFH and Competence in Coil insertion.
  • Evidence of CPD activities.
  • Minor surgery skills.

Experience:

  • Recent experience of working in general practice in the UK.
  • Commitment to and experience of working as part of a multi‐disciplinary and skill mixed team environment.
  • Experience of working to achieve standards within the Quality and Outcome Framework (QOF).
  • Evidence of independent working in General Practice.
  • Experience of supporting service change.
  • Evidence of participation in QOF.
  • Evidence of participation in Audit.
