I am a 23-yr old young man who has cerebral palsy, resulting in physical and learning disabilities requiring 24/7, 2:1 care. I currently have a support team of two staff, and we are in the process of recruiting more staff to support me at other times during the week and a day at the weekend. I wish to have motivated and experienced individuals to work alongside me, my family and rehabilitation team, to support and facilitate my independence.
I require support with all personal care, communication and mobility. I am a wheelchair user. Currently, my support team work with me mostly outside of the home engaging in therapy appointments, therapy programmes and enabling me to access community-based activities. I would like my Team Leader to be warm, patient and reliable, to encourage and support me to access all opportunities and help me to maximise my quality of life.
You will play a key part in helping to develop my day-to-day activities and I am looking for like-minded people to aid my rehabilitation further. As a Team Leader you will have the abilities to lead my support team by being organised at managing paperwork, ensuring staff rotas are up to date and supervising and appraising the care team. You will be professional when communicating with my therapists and family members and advocate on my behalf. You will be expected to contribute to the plans and risk assessments necessary for safe care to be delivered, ensuring these are up to date and reviewed on time. You will be an integral part of the rehab team.
As the Team Leader, you’ll have effective communication skills and be able to converse with me, my family and case manager when required. This role is subject to an Enhanced level DBS check and full induction and training will be provided.
ESSENTIAL INFORMATION – To join me as a Team Leader/Rehab Support Worker you will need to:
- Be able to drive and have use of a car.
- Have experience of supporting a person with cerebral palsy and a brain injury.
- Enjoy working as part of a team.
- Be willing to register with Social Care Wales.
