We are working closely with a Local Authority to appoint a Specialist Resettlement Coordinator on an interim basis. This role is vital in supporting single homeless applicants with complex needs, ensuring compliance with the Homelessness Reduction Act and delivering a person-centred, outcome-focused service.   You will be responsible for managing cases from prevention through to relief and final assessment, ensuring individuals have access to appropriate housing solutions and support services.   Key Responsibilities ·Deliver proactive, person-centred housing advice and casework, ensuring compliance with the Homelessness Reduction Act. ·Conduct comprehensive housing assessments, co-produce Personal Housing Plans (PHPs), and manage cases end to end. ·Assess applicants under prevention and relief duties, issuing S.184 decision letters and managing customer expectations. ·Work closely with partners to assess risk and create safeguarding plans for vulnerable applicants, including those leaving institutions such as prisons and hospitals. ·Minimise risk by identifying and reporting safeguarding concerns and ensuring appropriate follow-up. ·Collaborate with external agencies, including social services, health professionals, and support organisations, to improve housing outcomes. About You ·Extensive experience providing homelessness casework to single applicants with complex needs. ·Strong understanding of safeguarding policies, welfare, housing legislation, and case law. ·Experience working with applicants leaving institutions such as prisons or hospitals. ·Proven track record in achieving performance targets in a statutory homelessness setting. ·Ability to develop and implement Personal Housing Plans (PHPs) tailored to individual needs. ·Strong communication skills, with the ability to liaise effectively with applicants, stakeholders, and external agencies. ·Ability to work independently and within a team, prioritising a busy caseload. ·Knowledge of housing and welfare policies, including Universal Credit, tenancy rights, and private sector access schemes.
