Main purpose of the job role: The post holder will support vulnerable homeless adults. Their specific focus will be: Reducing repeat homelessness by supporting vulnerable adults, homeless people, former rough sleepers, offenders, and those with complex needs who are experiencing or at risk of homelessness. The post holder will assist service users in developing, utilizing, and enhancing both new and existing skills, knowledge, and confidence to break the cycle of homelessness, prevent its recurrence, and support sustained independent living. The post holder should be committed to supporting vulnerable service users as needed and must have a valid driving licence with access to a vehicle for work use.
Job description: Need to be on site 5 days per week.
Responsibilities:
- Complete outreach throughout the Walsall borough to locate rough sleepers, and follow up on all referrals received by the team.
- Undertake holistic risk and needs assessments and develop support and risk management plans.
- Support service users in enhancing their life skills by designing and implementing life and social skills programs within both individual and group work environments.
- Encourage service users to engage with learning skills and work.
- Encourage service users to develop positive family and other support networks.
- Assist service users to identify and take up settled accommodation and continue to provide ongoing support to them.
- Be available to service users for regular support sessions and respond to crises.
- Take responsibility for safeguarding the welfare of service users.
- Promote and facilitate the participation of service users in their individual support plans and in the management of the service.
- Facilitate service users' access to and take up of support services.
- Ensure benefit claims are made and maintained in order to maximise service users' income.
- Organise and accompany service users to appointments as appropriate.
