Location: Community-based (home visits and local settings)

Hours: Full-time – 36 hours

About the Role

We are looking for a caring and dedicated Intensive Dementia Outreach Worker to support adults living with dementia and their families in the community. You will work with people who have complex needs, helping them remain safe, supported, and independent in their own homes for as long as possible.

Key Responsibilities

  • Provide intensive, person-centred support to adults with dementia in their homes and community settings
  • Respond to crisis situations and help reduce the risk of hospital admission or placement breakdown
  • Build trusting relationships with individuals and their carers
  • Support people with daily living, emotional wellbeing, and maintaining routines
  • Work closely with families, carers, health professionals, and social care teams
  • Carry out assessments, create support plans, and keep clear, accurate records
  • Promote dignity, respect, and choice at all times

Skills and Experience

  • Experience supporting adults with dementia or other cognitive impairments
  • Ability to remain calm, patient, and compassionate in challenging situations
  • Good communication skills and the ability to build rapport
  • Experience working in community, outreach, or crisis support services is desirable
  • Ability to work independently and as part of a multidisciplinary team

  • Willingness to travel within the community

  • Training: Fire Safety — Within Last: 1 year
  • Training: Basic Life Support — Within Last: 1 year
  • Training: First Aid one day (Emergency First Aid) — Within Last: 1 year
  • Training: Health & Safety — Within Last: 3 years
  • Training: Infection prevention & Control — Within Last: 3 years
  • Training: Mental Capacity Act including DoLS — Within Last: 3 years
  • Training: Moving and Handling People — Within Last: 3 years
