Purpose of the Role
To provide high-quality information, advice, guidance, and practical support to care-experienced young people aged 16–25, enabling them to make a successful transition into independent adulthood. Build positive, trusting relationships and coordinate services to ensure young people achieve positive outcomes across accommodation, education, employment, health, finances, wellbeing, and independent living.
Key Responsibilities
- Build and maintain strong, positive relationships with care-experienced young people, acting as a consistent source of support and guidance.
- Develop, implement, monitor, and review Pathway Plans that reflect each young person’s strengths, aspirations, needs, and goals.
- Use the Signs of Safety and Valuing Care frameworks to assess needs, manage risks, and support effective planning.
- Provide tailored information, advice, guidance, and practical support to help young people develop independence.
- Support young people with securing and maintaining suitable accommodation.
- Assist young people in accessing education, employment, apprenticeships, and training opportunities.
- Promote physical, emotional, and mental wellbeing by facilitating access to appropriate health services and support.
- Support young people in developing financial capability, including budgeting, benefits, and managing living expenses.
- Teach and encourage independent living skills such as cooking, household management, and accessing community resources.
- Conduct statutory face-to-face visits in accordance with legislative and organisational requirements.
- Identify, assess, and manage safeguarding concerns, vulnerabilities, and risks, taking appropriate action to protect young people.
- Coordinate and work collaboratively with internal teams and external agencies, including housing, education, health professionals, social workers, and voluntary organisations.
- Act as the Lead Professional where appropriate, ensuring coordinated, person-centred support.
- Deliver targeted interventions that improve outcomes and reduce barriers to independence.
- Participate in the duty rota, providing timely support to young people experiencing crisis situations.
- Maintain accurate, timely, and confidential case records and reports.
- Travel locally and nationally to meet young people and attend meetings as required.
Knowledge & Skills
- Ability to build trusting and effective professional relationships with vulnerable young people.
- Understanding of safeguarding legislation, risk assessment, and child and adolescent development.
- Knowledge of issues affecting care-experienced young people and the challenges associated with leaving care.
- Ability to develop person-centred support plans and monitor progress.
- Strong communication, advocacy, and interpersonal skills.
- Experience working collaboratively within multi-agency environments.
- Good organisational, planning, and time management skills.
- Ability to manage a varied caseload and respond effectively to changing priorities.
- Competent IT and case management record-keeping skills.
Experience
- Experience supporting young people aged 16–25, particularly those with complex or vulnerable needs.
- Experience delivering practical, emotional, and person-centred support.
- Experience working in partnership with statutory, voluntary, and community organisations.
- Experience assessing needs, managing risk, and supporting positive outcomes.
Qualifications
- NVQ Level 3 (or equivalent) in Health & Social Care, Youth Work, Childcare, or a related field (preferred).
- Full UK driving licence.
- Access to a vehicle and willingness to travel locally and nationally.
