Post Grade: 9 (31-35)
Responsible to: Occupational therapy and specialist assessment team manager.
Responsible for: Assessment of adults with a physical disability, sensory impairment and/or learning disability. Work includes assessing for aids to independence, works of adaptation (major and minor), manual handling assessments and evaluating the effectiveness/impact of all aforementioned. Promoting independence to avoid the need for citizens to depend on care. Helping the Council keep people at home for as long as possible and keeping costs of care packages down.
Liaison: Liaising with colleagues in the NHS, social services, other Council directorates and voluntary organisations where applicable. A patch system is in operation and effective working relationships with members of the primary health care team and GPs will be expected. Occupational Therapy and other students are an integral part of the team. Fieldwork Education will be undertaken and the post holder may lead or support in this process.
Major Purpose of Post
To promote and maximise independence, social inclusion and safe care for all clients on caseload.
Main Duties and Responsibilities
- To carry out occupational therapy assessments, intervention and evaluation of service provision for adults (primarily those with complex needs).
- To work in the context of an interdisciplinary setting and to liaise with and provide advice to team members and social work colleagues as required.
- To liaise with and work in conjunction with other agencies as required.
- To effectively monitor the progress of recommendations for works of adaptation and equipment. This includes review of provisions as required.
- To support and supervise the work of junior staff students and Occupational Therapy Assistants as delegated by the Team Manager so as to ensure consistent standards of good practice and activity within the team.
- To complete administrative tasks relevant to the caseload, in particular ensuring that the computer-based data system is properly populated to evidence activity, assessments and services. Case notes will be contemporaneous with activity.
- To contribute towards the development of services to adults in the Vale of Glamorgan Council catchment area.
- To prepare for and attend supervision with the team manager or authorised designate.
- To attend team meetings and to lead and contribute to this as applicable.
- Ensure compliance with the Council’s Financial Regulations, Standing Orders, Policies and Procedures.
- To implement the principles of the Council’s Equal Opportunity Policy whilst carrying out the above duties.
- To adhere to Health and Safety legislation / relevant Council policies and procedures and to take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts / omissions.
- To implement the principles of the Council’s Environment Policy while carrying out the above duties.
- Such other duties and responsibilities commensurate with the grade and in accordance with the general character of the job as may reasonably be required by the Chief Officer from time to time.
