Job Title: Care Assistant

  • Full time
  • Β£30,060 a year

Job Purpose:

Assist as a team member in the effective assessment, implementation, and review of individual care plans for service users in the Centre or in the Community.

Principal Responsibilities:

  1. Maintain a warm, caring, and stimulating environment where service users receive good physical, emotional, social, and intellectual care.
  2. Act as a key worker for a number of service users, working in partnership with them and relevant persons to identify needs and coordinate/implement their individual care plans.
  3. Contribute to the assessment, implementation, recording, and reviewing of individual care plans to the standards specified in the service.
  4. Maintain effective record systems to support the preparation of Social Work reports for Court Reviews, Planning Meetings, etc.
  5. Liaise with other professional staff and external parties to identify and mobilize appropriate resources for the benefit of the individual.
  6. Comply with statutory requirements, group policies, equal opportunities, and other regulations. Review service user needs continuously and recommend improvements to achieve service objectives.

Note:

The job holder will undertake such work as determined by the Manager, up to or at a level consistent with the Principal Responsibilities of the job.

This role is a flexible worker style, working at effective locations and times, respecting task, customer, individual, and team needs. Services and customers span across Cheshire West and Chester, so you may be required to work at any location within the borough.
