As a Minor Illness Nurse, the postholder will demonstrate courageous and safe, critical thinking and is a caring, compassionate and committed experienced individual who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, with support of their mentor.

Main duties of the job
Working within their scope of practice and access suitable support.
Being compliant with the Carn to Coast clinical framework.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with minor illness symptoms.
Proactively identify, diagnose, and refer patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage minor illness conditions, integrating treatment methods into a management plan.
Consider medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
Work with patients in order to support compliance with and adherence to prescribed treatments.
Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

Carn to Coast Health Centres is a 9-Partner Cornish GP Practice serving over 29,000 patients. We have a main surgery in Pool and branch surgeries in Illogan and Camborne.
Our Practice has historically been at the forefront in establishing a highly skill-mixed approach and this has been extended by maximising the use of additional roles within the North Kerrier West PCN (comprising our Practice and the neighbouring Praze Surgery). The mix now includes paramedics, mental health staff, clinical pharmacists and technicians, care co-ordination and physiotherapy, all supported by a substantial administrative team, whose role includes processing clinical correspondence and workflow (through Docman). Our Klinik system uses AI to support our clinicians in triaging incoming contacts appropriately so that patients are engaged and brought in as appropriate.

Person Specification
Knowledge
- Local and national health policy awareness.

Qualifications
- Active registration with appropriate governing body (e.g. NMC, HCPC).
Experience
- Knowledge and experience in the assessment management of both adults and paediatrics.
- Clinical experience in the area General Practice.
- Assessment and management of minor illness.
Skills
- Clinical examination skills including history note taking, assessment and treatment planning.
- Communication skills, both written and verbal.
- Good level of IT Skills.
- Current revalidation.
- Clinical leadership skills
- Teaching and mentorship in clinical setting.
- Communication of difficult conversations/messages to patients and families.
- Willingness to undertake further training.
