We are seeking a dedicated and compassionate Social Worker to join our Adults Community team in Worcestershire. This role is pivotal in supporting adults facing various challenges, ensuring they receive the care and assistance they need to lead fulfilling lives. As a Social Worker, you will engage with clients, assess their needs, and develop tailored support plans while working collaboratively with families and other professionals.
Your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs and circumstances.
- Developing, implementing, and reviewing care plans in partnership with clients and their families.
- Providing emotional support and practical advice to clients and their families.
- Collaborating with multidisciplinary teams and external agencies to coordinate care and support services.
- Maintaining accurate and up-to-date records in accordance with policies and procedures.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with adults in a community setting.
- Strong communication and interpersonal skills, with the ability to build rapport with clients.
- Excellent organizational skills and the ability to manage a caseload effectively.
