As a member of our Locality Team, you will be responsible for a range of key duties, including:

  • Conducting comprehensive assessments of individuals’ needs and risks.
  • Developing and implementing tailored support plans in collaboration with clients and their families.
  • Providing ongoing support and guidance to clients, ensuring their voices are heard and respected.
  • Collaborating with various stakeholders, including healthcare providers, community organizations, and legal entities.
  • Documenting case notes and maintaining accurate records in accordance with policy and legislation.

The ideal candidate will possess essential qualifications and skills, including:

  • A recognized social work qualification (e.g., degree in social work or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, preferably within a locality team.
  • Strong assessment and analytical skills, with the ability to work under pressure.
  • Excellent communication and interpersonal skills, with a compassionate approach to client care.
