The City of Wolverhampton is seeking a dedicated and compassionate Qualified Social Worker to join our Ageing Well Team.
This role is vital in supporting older adults in our community to live fulfilling and independent lives.
As a key member of our team, you will work collaboratively with individuals, families, and various stakeholders to provide high-quality social care services that promote wellbeing and safeguard vulnerable adults.
Key responsibilities of the role include
- Conducting assessments of individuals' needs and developing tailored support plans.
- Providing direct support and guidance to older adults and their families, ensuring their voices are heard.
- Collaborating with healthcare professionals, community organizations, and other agencies to coordinate care and resources.
- Monitoring and reviewing care plans to ensure they remain effective and relevant.
- Promoting independence and access to community resources, including social activities and support services.
- Maintaining accurate and up-to-date case records in compliance with local policies and procedures.
To be considered for this position, candidates must possess the following essential qualifications and skills:
- A recognized Social Work qualification (e. g., Degree in Social Work, Dip SW, or equivalent).
- Registration with Social Work England.
- Experience in adult social care, particularly with older adults.
- Strong assessment and analytical skills, with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, demonstrating empathy and understanding.
