Wokingham Borough Council is seeking an Adult's Social Worker to join our Older People's Team.

In this role, you will play a crucial part in supporting vulnerable adults, ensuring they receive the care and assistance needed to enhance their quality of life.

Your expertise will be vital in assessing needs, developing care plans, and coordinating services to promote independence and well-being.

Your key responsibilities will include

  • Conducting comprehensive assessments of older adults’ needs and preferences.
  • Developing and implementing personalized care plans in collaboration with clients and their families.
