The South and West Herts Older People & Sensory Services Team is seeking a dedicated and compassionate Qualified Social Worker to join our dynamic team. Key responsibilities include:
- Conducting comprehensive assessments of clients' needs and developing tailored care plans.
- Collaborating with healthcare professionals, families, and community organizations to coordinate services.
- Providing ongoing support and guidance to clients and their families, ensuring they understand available resources.
- Monitoring and reviewing care plans regularly to adapt to clients' changing needs.
- Advocating for clients' rights and well-being, ensuring their voices are heard in decision-making processes.
To be considered for this position, candidates must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registered with Social Work England.
- Experience working with older adults and/or individuals with sensory impairments.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and families.
- Excellent organizational skills and the ability to manage a caseload effectively.
