As a member of our team, your key responsibilities will include:

  • Conducting assessments to identify the needs and aspirations of young adults and their families.
  • Developing and implementing personalized support plans that promote independence and well-being.
  • Facilitating access to education, employment, and social opportunities.
  • Collaborating with multi-disciplinary teams to ensure holistic support for individuals.
  • Providing advice, guidance, and advocacy to young adults and their families.
  • Maintaining accurate case records and documentation in compliance with local authority policies.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Registered with Social Work England.
  • Experience working with young adults, particularly those with disabilities or special educational needs.
  • Strong communication and interpersonal skills, with the ability to build rapport with young people and their families.
  • Knowledge of relevant legislation and best practices in social work.
  • Excellent organizational skills and the ability to manage a diverse caseload.
