• Conducting comprehensive assessments to identify the needs of adults within the community.
• Developing, implementing, and reviewing personalised care plans in collaboration with service users.
• Providing support and guidance to individuals and their families, ensuring their voices are heard.
• Working closely with multidisciplinary teams to ensure holistic care and support.
• Maintaining accurate records and documentation in accordance with regulatory requirements.

To be considered for this role, you must possess the following qualifications and skills:

• A recognised Social Work qualification (e.g., BA/MA in Social Work).
• Registration with Social Work England.
• Demonstrable experience in adult social work, particularly in community settings.
• Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
• Excellent organisational skills and the ability to manage a varied caseload effectively.
