We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Learning Disabilities (LD) Team in Walsall. As a key member of our team, you will play a vital role in delivering high-quality social work services to adults with learning disabilities and their families.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and preferences of individuals with learning disabilities.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to individuals, helping them access appropriate services and resources.
- Working closely with multidisciplinary teams, including health professionals, to ensure holistic care.
- Maintaining accurate and up-to-date case records in accordance with statutory requirements and best practice guidelines.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work or equivalent).
- Current registration with Social Work England.
- Experience working with adults with learning disabilities and knowledge of relevant legislation and policies.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
- Excellent problem-solving abilities and a proactive approach to overcoming challenges.
