Key responsibilities include:

  • Conducting comprehensive assessments of adults' needs and circumstances.
  • Developing and implementing person-centered care plans in collaboration with service users and their families.

  • Maintaining accurate records and documentation in accordance with local and national standards.

The ideal candidate will possess:

  • A relevant social work qualification (e.g., BA in Social Work, MA in Social Work, or equivalent).
  • Registration with Social Work England.
  • Experience in adult social care, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
