- Leading assessments and reviews for individuals eligible for Continuing Healthcare funding.
- Developing and implementing person-centered care plans in collaboration with clients and their families.
- Providing expert guidance and support to junior social workers and other team members.
- Ensuring compliance with relevant legislation and best practices in social work.
- Engaging with external agencies and stakeholders to promote integrated care solutions.
The ideal candidate will possess the following qualifications and skills:
- Registered Social Worker with a valid Social Work England registration.
- A relevant degree in Social Work or equivalent qualification.
