The Leaving Care Team in Hackney is seeking a qualified Social Worker to join our dynamic team. In this role, you will work closely with young adults aged 16-25, providing support and guidance as they navigate the challenges of independent living.

Your key responsibilities will include:

  • Conducting assessments and developing personalized care plans for young people.
  • Providing direct support and advocacy to ensure the best outcomes for each individual.
  • Collaborating with external agencies, including housing services and educational institutions.
  • Facilitating life skills training and workshops to empower young adults.
  • Maintaining accurate and up-to-date case records in line with local and national standards.

To be successful in this role, you must possess the following essential qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, CQSW).
  • Current registration with Social Work England.
  • Experience in working with young people, particularly in a leaving care context.
  • Strong communication and interpersonal skills, with the ability to build rapport.
  • Excellent organizational skills and the ability to manage a diverse caseload effectively.
