We are seeking a dedicated and compassionate Social Worker to join our Learning Disability Team in Harrow. Your key responsibilities will include:
- Conducting assessments of individuals with learning disabilities to identify their needs and support requirements.
- Developing and implementing personalized care plans in collaboration with clients, families, and other professionals.
- Providing ongoing support and advocacy to clients and their families, helping them navigate available resources and services.
- Working closely with multidisciplinary teams to ensure a holistic approach to care and support.
- Maintaining accurate and up-to-date case records in compliance with statutory requirements and organizational policies.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work or equivalent).
- Current registration with Social Work England.
- Experience working with individuals with learning disabilities and their families.
- Strong communication and interpersonal skills, with the ability to build rapport and trust.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
