We are seeking a qualified Social Worker to join our dynamic team in Blackpool.
In this role, you will provide essential support and advocacy for clients navigating complex health care needs, ensuring they receive the appropriate resources and care to enhance their quality of life.
Your key responsibilities will include
- Conducting comprehensive assessments to determine the health and social care needs of clients.
- Developing and implementing care plans in collaboration with clients, families, and multidisciplinary teams.
- Providing ongoing support and guidance to clients and their families throughout the care process.
- Advocating for clients' rights and access to necessary services and resources.
- Maintaining accurate records and documentation in compliance with relevant policies and procedures.
- The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (e. g., BA/MA in Social Work).
- Current registration with Social Work England.
- Proven experience in social work, particularly in continuing health care or a related field.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Excellent organizational skills and the ability to manage a diverse caseload effectively.
