Your expertise will be crucial in managing a diverse caseload and addressing the unique needs of our clients.
Your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs and circumstances.
- Developing and implementing tailored care plans in collaboration with clients and relevant stakeholders.
- Monitoring and reviewing clients' progress, adjusting care plans as necessary.
- Providing emotional support and practical advice to clients and their families.
- Coordinating with other professionals and agencies to ensure holistic care.
- Maintaining accurate and up-to-date case records in compliance with legislation and best practices.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Proven experience in case management within a social work setting.
- Strong assessment and analytical skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
