We are seeking a dedicated and compassionate Social Worker to join our Case Management team in Hackney, focusing on backlog cases. In this challenging yet rewarding role, you will be responsible for providing high-quality social work services to individuals and families, ensuring they receive the support and resources they need to thrive. Your expertise will be crucial in managing a diverse caseload and addressing the unique needs of our clients.

Your key responsibilities will include:

  • Conducting comprehensive assessments of clients' needs and circumstances.
  • Developing and implementing tailored care plans in collaboration with clients and relevant stakeholders.
  • Monitoring and reviewing clients' progress, adjusting care plans as necessary.
  • Providing emotional support and practical advice to clients and their families.
  • Coordinating with other professionals and agencies to ensure holistic care.
  • Maintaining accurate and up-to-date case records in compliance with legislation and best practices.

The ideal candidate will possess the following qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience in case management within a social work setting.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently and as part of a multidisciplinary team.
