Your key responsibilities will include:
- Conducting comprehensive assessments to determine the needs of adults with disabilities.
- Developing and implementing personalized care plans in collaboration with service users and their families.
- Providing ongoing support, guidance, and advocacy to individuals, ensuring their voices are heard.
- Working closely with multi-disciplinary teams and external agencies to coordinate services and resources.
- Maintaining accurate and up-to-date case records in compliance with legal and organizational standards.
The ideal candidate will possess the following essential qualifications and skills:
- A degree in Social Work or equivalent qualification recognized by Social Work England.
- Registration with Social Work England.
- Experience working with adults with disabilities or within a similar social work setting.
- Strong communication and interpersonal skills, with the ability to build effective relationships.
- Excellent organizational skills and the ability to manage a caseload effectively.
