About the Role: We are seeking an experienced Social Worker to join the 0-25 Together Service, supporting children with disabilities and their families. This is a maternity cover role requiring strong assessment and case management skills, with a focus on safeguarding and multi-agency collaboration.
Key Responsibilities:
- Receive and investigate referrals for children with disabilities and complete assessments.
- Assess safety and risk, developing safety plans where necessary.
- Prepare paperwork for resource allocation panels.
- Work collaboratively with agencies including schools, mental health providers, and legal teams.
- Maintain accurate documentation and produce clear reports for decision-making.
- Provide practical advice on long-term implications of care plans.
What We're Looking For:
- Qualified Social Worker with minimum 3 years' post-qualifying experience.
- Strong assessment and court experience.
- Ability to work collaboratively with multi-agency teams.
- Must be a car driver with access to own transport.
