We are seeking a dedicated and compassionate Social Worker to join our Acute Hospital team in Shropshire.
This role is vital in providing support to patients and their families during challenging times, ensuring they receive the best possible care and assistance.
As a Social Worker in our acute hospital setting, you will work collaboratively with healthcare professionals to assess and address the psychosocial needs of patients, facilitating a smooth transition from hospital to home or other care settings.
Key responsibilities include
- Conducting comprehensive assessments to identify patients' social, emotional, and practical needs.
- Developing and implementing individualized care plans in partnership with patients, families, and multidisciplinary teams.
- Providing emotional support and counseling to patients and their families throughout their hospital stay.
- Coordinating discharge planning and connecting patients with community resources and services.
- Advocating for patients' rights and ensuring they understand their options for care and support.
- To be considered for this role, you must possess the following qualifications and skills:
- A recognized Social Work qualification (e. g., degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience in a healthcare setting, preferably in acute hospital environments.
- Strong communication and interpersonal skills, with the ability to work effectively in a team.
- Empathy and resilience, with a genuine passion for supporting vulnerable individuals.
