Your key responsibilities will include:
- Conducting comprehensive assessments of individuals’ social care needs and risks.
- Developing and implementing personalized care plans in collaboration with healthcare professionals, patients, and their families.
- Providing ongoing support and advocacy for patients, ensuring their voices are heard in care decisions.
- Coordinating discharge planning and connecting patients with community services and resources.
- Maintaining accurate and up-to-date records in accordance with legal and organizational standards.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognised Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience working within adult social care, preferably in a hospital setting.
- Strong assessment and analytical skills, with the ability to work under pressure.
