- Providing practical and emotional support to individuals and their families.
- Collaborating with multidisciplinary teams to coordinate services and interventions.
- Monitoring and reviewing care plans to ensure effective service delivery.
- Advocating for the rights and needs of service users in various settings.
The ideal candidate will possess the following essential qualifications and skills:
- A relevant qualification in Social Work or equivalent (e.g., DipSW, CQSW, or CSS).
- Registered with Social Work England or eligible for registration.
- Demonstrable experience in adult social care or community support.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
- Excellent organizational skills and the ability to work independently as well as part of a team.
