Social Worker - Mental Health Hospital Discharge - Sheffield

We are seeking a dedicated and compassionate Social Worker to join our team at a leading Mental Health Hospital in Sheffield. This role focuses on facilitating smooth and effective discharges for patients, ensuring they receive the support needed to transition back into the community. You will work closely with multidisciplinary teams, patients, and their families to develop tailored discharge plans that promote mental well-being and recovery.

Key responsibilities include:

• Conducting comprehensive assessments to identify patient needs and suitable post-discharge services.
• Collaborating with healthcare professionals to coordinate care plans and follow-up services.
• Providing emotional support and guidance to patients and their families throughout the discharge process.
• Advocating for patients’ rights and ensuring they have access to necessary resources.
• Maintaining accurate and confidential records of patient interactions and discharge plans.

To be considered for this role, candidates must possess:

• A recognized qualification in social work (Degree in Social Work or equivalent).
• Registration with the Health and Care Professions Council (HCPC).
• Experience working in mental health settings, particularly in discharge planning.
• Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
• Excellent organizational skills and the ability to manage multiple cases effectively.
