Join our dedicated Adulthood Team in Rotherham as a Qualified Social Worker, where you will play a vital role in supporting adults to lead fulfilling lives and achieve their goals. In this position, you will work collaboratively with individuals, families, and multi-agency partners to assess needs, develop care plans, and implement support strategies tailored to each person’s circumstances. Your expertise will be crucial in promoting independence and safeguarding vulnerable adults within our community.
Your key responsibilities will include:
- Conducting comprehensive assessments of individuals’ needs and risks.
- Developing and reviewing care plans in collaboration with service users and their families.
- Providing direct support and guidance to adults with varying needs, including mental health issues, learning disabilities, and physical disabilities.
- Maintaining accurate and up-to-date records in compliance with local authority policies and procedures.
- Participating in multidisciplinary meetings and contributing to the continuous improvement of services.
The ideal candidate will possess a relevant social work qualification (e.g., CQSW, DipSW, or equivalent) and be registered with Social Work England. You should have a solid understanding of the Care Act 2014 and related legislation, alongside practical experience in adult social work. Strong interpersonal and communication skills are essential, as well as the ability to work effectively in a team and manage a diverse caseload.
