Your key responsibilities will include:
- Conducting assessments to identify the needs and strengths of individuals.
- Developing and implementing person-centered care plans in partnership with clients and their families.
- Providing ongoing support, guidance, and advocacy to clients, ensuring their voices are heard.
- Collaborating with multi-disciplinary teams and external agencies to deliver integrated services.
- Monitoring and reviewing care plans to ensure effectiveness and adjust as necessary.
To be considered for this role, you must possess the following essential qualifications and skills:
- A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Demonstrable experience in adult social work, preferably within a community setting.
- Strong communication and interpersonal skills, with the ability to build rapport with clients.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
