We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Learning Disabilities Team in Cambridgeshire.

As a vital member of our team, you will engage with clients, their families, and other professionals to provide comprehensive assessments and tailored support plans.
Your key responsibilities will include
- Conducting thorough assessments of individuals with learning disabilities to identify their needs and preferences.
- Developing and implementing person-centered support plans that promote independence and well-being.
- Collaborating with multi-disciplinary teams, including healthcare professionals and community organizations, to ensure holistic support.
- Providing ongoing monitoring and review of support plans, adapting them as necessary to reflect changes in circumstances.
- Advocating for clients’ rights and ensuring they have access to appropriate resources and services.
- The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (BA/MA in Social Work or equivalent).
- Current Social Work England registration.
- Proven experience working with adults with learning disabilities or similar client groups.
- Strong assessment and analytical skills, with the ability to create effective support plans.
- Excellent communication and interpersonal skills, with a commitment to person-centered practice.
