Hospital Discharge / Discharge to Assess (D2A) - Social Worker - Nottingham

We are seeking a dedicated and compassionate Hospital Discharge / Discharge to Assess (D2A) Social Worker to join our dynamic team in Nottingham. This role is pivotal in facilitating smooth transitions for patients from hospital to home or appropriate care settings. As a key member of our multidisciplinary team, you will work collaboratively with healthcare professionals, patients, and their families to ensure that discharge plans are executed effectively and in the best interest of our clients.

Your key responsibilities will include:

• Conducting assessments to determine the needs of patients and their families during the discharge process.
• Developing and implementing personalized discharge plans that promote independence and well-being.
• Coordinating with external agencies and community resources to arrange post-discharge support services.
• Providing emotional support and guidance to patients and their families throughout the discharge process.
• Documenting assessments, plans, and outcomes in compliance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., Degree in Social Work or equivalent).
• Current registration with Social Work England.
• Experience in hospital discharge planning or community social work.
