We are seeking a dedicated and experienced Adult Social Worker to join our Assessment Team in Nottingham. This is an exciting opportunity for a qualified professional eager to make a meaningful impact in the lives of vulnerable adults. The successful candidate will play a crucial role in assessing needs, developing care plans, and ensuring the delivery of high-quality services to support individuals in achieving their desired outcomes.

Key responsibilities include:

  • Conducting comprehensive assessments of adults in need of care and support.
  • Developing and implementing personalized care plans in collaboration with service users and their families.
  • Working closely with multidisciplinary teams to ensure coordinated service delivery.
  • Monitoring and reviewing care plans to adapt to changing needs and circumstances.
  • Providing advice and guidance to individuals and their families regarding available services and entitlements.

The ideal candidate will possess the following qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations.
  • Excellent organizational skills and the ability to manage a varied caseload.
